Steps

  1. Inside your SharePoint site, go to the left-side panel and click the site library where you want to create the form.
  2. Click New to open the content creation menu.
  3. Select Forms Survey from the list of options.

  5. Click Quick Start With to proceed to the form builder.
  6. Choose your preferred question type and fill in the required question details in the input fields provided.
  7. Click Add New Question and repeat the steps to add more questions to your form.
  8. Click Collect Responses to save your form and send it to the intended recipients.
