This tutorial shows how to create an intake form in Smartsheet by configuring column properties in Grid View and using the built-in Forms builder to collect structured information from participants. Once saved, the form can be instantly shared with respondents and responses flow directly into your sheet.
Steps
- In Smartsheet's Grid View, locate the column header and click the Column Menu button to begin configuring column properties.
- Select Edit Column Properties from the dropdown menu.
- Enter the column name in the input field and specify the corresponding column type.
- Click OK to apply and save the column changes.
- Repeat the column configuration steps for each additional column until all required fields are set up.
- Navigate to the top menu bar and click Forms.
- Choose Create Form from the options to open the form builder.
- Enter a unique form title and description in their respective fields and customize their formatting as needed.
- Click each form field one by one to select and configure it.
- Use the right-side panel to add a Label, Help Text, and adjust any related field settings.
- Once all fields are configured, click Save in the top-right corner to finalize the form.
- Your Smartsheet intake form is now created — share it with intended participants to begin collecting information.
